New Patient Information

At Conrad Petersen Dermatology, we are here to provide excellent dermatology care in a safe environment for our patients and staff.

If you are a NEW PATIENT, please read below:

  • You will receive an email entitled “Action Required.” It is mandatory that you enter your complete medical history and pharmacy information into the Patient Portal. Please note: We will not allow patients to be seen if they have not completed their online Patient Portal registration in entirety, and rescheduling will be required.
  • Our iPads for check-in are temporarily not available for patient registration in office due to increased infection precautions.
  • All consents and office policies are emailed to you in advance and require signatures. Please sign and email back to: info@conradpetersen.com
  • All financial transactions will be processed electronically. Your credit card information will be collected in advance and stored in a secured online portal.

Patient Precautions

  • Please contact us if you display any signs of illness in the last 14 days (fever, chills, cough, headache, fatigue, loss of taste/smell) or if you have been in contact with someone who was diagnosed with COVID-19 or have been asked to quarantine. These circumstances require rescheduling your appointment. 
  • Please come to your appointment alone. No visitors are allowed except for one parent for minors or one visitor for patients requiring assistance for disability. 
  • Patients are required to wear a mask. We will be taking touchless temperatures of all patients at the time of check-in.
