Parking Instructions

Parking Instructions

Conveniently located near Lincoln Park, the Presence Center for Advanced Care building has its own parking garage attached to the building. Follow AMITA St. Joseph signs directing to the number 4. The entrance to the garage is located off Commonwealth Avenue, which can be accessed via either Surf Street or Diversey Parkway. Upon entering the garage you will receive a ticket which you may validate for $5 before or after your appointment at the lobby front desk on the first floor. Our office is Suite 700, located on the 7th floor.

Missed appointments / Cancellation Fee: 

The following no-show fees affect all patients who do not keep their scheduled appointment or who cancel an appointment with less than a 24-hour notice:

$30 general dermatology appointment

$50 cosmetic appointment

$150 surgery appointment

Patients who fail to show or cancel / reschedules an appointment without 24 hour notice will be charged the appropriate no-show fee. We will charge the credit card on file and email a receipt.

No future appointment can be scheduled until no show fees are paid.
